Lens of Grace Advisory is a division of Lens of Grace LLC, a minority-owned business enterprise (MBE) located in South Bend, Indiana. We provide executive coaching, organizational consulting, leadership advisory, and related services to healthcare organizations, nonprofits, and individual leaders. This Privacy Policy governs our website at lensofgraceadvisory.com and any services accessed through it.

We collect the following categories of information when you interact with our website or services:

Personal identifiers: Your name, email address, phone number, and organization name — collected when you submit a contact form, book a consultation, or complete the Organizational Health Snapshot assessment.

Assessment data: If you complete the Organizational Health Snapshot, we collect your self-reported responses, category scores, and total score. This data is directional and self-reported; it does not constitute a professional diagnosis or evaluation.

4. Coaching and Consulting Confidentiality

Information shared during coaching or consulting engagements is treated as confidential and is not disclosed to third parties without your explicit consent, except: (1) when required by law; (2) when there is imminent risk of harm to self or others; or (3) when used in anonymized form for case studies or testimonials, with prior written approval.

Coaching confidentiality is not equivalent to therapist-client privilege and does not carry the same legal protections under Indiana or federal law. This distinction is disclosed in all engagement agreements.

Yvonne Kaunda's clinical internship placements are conducted under licensed supervision and are entirely separate from her Lens of Grace Advisory coaching practice — separate clients, separate documentation, and separate scope. Information shared in coaching sessions is not part of any clinical record.

5. Protected Health Information (PHI)

Lens of Grace Advisory does not collect, store, transmit, or process protected health information (PHI) as defined under the Health Insurance Portability and Accountability Act (HIPAA) through this website or its contact forms.

Do not submit PHI — including patient names, medical record numbers, diagnosis information, treatment details, or any individually identifiable health information — through any form, email, or assessment tool on this site.

Our organizational consulting services are operational and advisory in nature. We do not function as a covered entity or business associate under HIPAA unless a formal Business Associate Agreement (BAA) has been separately executed in writing.

We may use the following third-party services that have access to limited user data:

We retain personal information for as long as necessary to fulfill the purposes for which it was collected, or as required by law. Assessment data and inquiry information is typically retained for a period of 24 months from the date of submission, after which it is deleted from our systems. Engagement records for active or past clients are retained in accordance with applicable professional and legal standards, typically for a period of seven years or as otherwise required by law. Coaching and consulting engagement records are retained in accordance with applicable professional and legal standards.
